Make Adding Taxonomy/Provider Specialty Codes Optional On Claims Depending on Insurer Requirements

Not all insurers require taxonomy/provider specialty codes, so it would be helpful to be able to select which insurance claims we'd like the taxonomy codes to appear on. Without this, we have to manually delete the code from some claims.
